概要: 幻覚とは

定義 definition

幻覚 hallucination は 実際の刺激がない状態で、あるように知覚すること と定義される。幻視 visual hallucination と幻聴 auditory verbal hallucination が有名であるが、5 感以外にも様々な感覚 (痛み、温度など) で起こる (2)。

  • Hallucinations can be defined as perceptions in the absence of external stimuli (1I).
  • Hallucinations can be defined as perceptions without corresponding sources in the external world (3I).
  • Hallucinations can be defined as percepts, experienced by a waking individual, in the absence of appropriate stimuli from the extracorporeal world (2).

「この病気の **% が幻覚を経験している」というようなデータが示されることがあるが、これは患者への聞き取り調査によるものであり、割合を underestimate している可能性が高い。


State study と trait study

Hallucination に関する脳科学研究には、大きく分けて state と trait の2通りの比較方法がある (4I)。
    : State は、患者が hallucination を見て/聞いているときといないときの比較。
    : Trait は、hallucination を見たことがある/見やすい患者と、そうでない患者の比較。

幻覚と相関する脳の形態異常

統合失調症の場合

幻覚と脳の形態の関係はあまり明瞭なものではなく、個人レベルの解析では見つからないことが多い (5)。しかし、グループで解析すると差がみられる。

> 統合失調症患者では、幻聴は languadge regions の形態異常と相関する (5)。

: ROI analysisで、left superior temporal gyrus (STG) の容積低下が確認されている。
: 全自動の voxel-based whole brain morphometry でも、STG の容積低下が示された。
: STG の他、視床 thalamus や 小脳 cerebellum の容積も低下している。

> 言語機能に関わる大脳皮質の折りたたみ cortical gyrification も、幻聴に伴い低下している。

: Gyrification は脳の発生に影響される指標の一つ。統合失調症の遠因が発生過程にあることを示唆。
